Administering Medication
At Goodrich Primary School, we do administer medication but this is under the direction of the parent / carer. We can only administer the medication which we have a signed permission form for as outlined below.
If your child has a short term prescribed medication we will need you to complete a form (Form 1) before you hand in the medicine to be administered, to the school office.
If your child has long term prescribed medication we will need you to complete two forms (Form 1 and 2) before you hand in the medicine to be administered, to the school office.
If your child suffers from asthma, please complete all 3 forms below.
If your child has more than one medication please complete form 1 for each medication.
